Form W-9 is used to officially ask a person or business to provide their name, address, and taxpayer identification number so that the Free Methodist Church of North America can properly issue tax documents to the IRS. In general, the Form W-9 is used if you are paid interest, dividends, non-employee compensation, and certain other types of reportable income for which you would receive a 1099 form. If you are receiving payments for which you would generally expect to receive a 1099 form, please complete and return the W-9 form to the Human Resources Department. In the section marked “List account number(s) here (optional)” please note what type of work you are performing, or for which department you are working. Thank you.
