Are you ready for retirement? If you are 100 percent vested, and at least age 60, you may begin your retirement benefits as soon as you are ready. Normal retirement age is 65. However, you may elect to receive your benefits as early as age 60, at a reduced amount.
If you would like to apply for retirement benefits at this time, please complete the retirement application. You may do this by filling out the form below, or by printing out the application and mailing or faxing it to our office. If you would like to print the application out, click on the link below.
Please note: We will need a copy of both your and your beneficiary’s birth certificates or driver’s licenses. Please mail or fax these to our office, even if you apply online.
After the completed retirement application is received by our office, a final benefit calculation will be sent to you. You will then have the opportunity to select the benefit option desired and note when the benefit should begin. In addition, we will send other forms to you to complete once we receive the application, such as a housing allowance form, a W4-P tax form and a direct deposit form.
